The Thr1ve Social Prescribing Team

We are a team of specialist link workers who understand the challenges young people face in today’s world, and we recognise their potential. Our mission is to support young people who want to make positive changes in their lives.

We offer an award-winning social prescribing service, providing both 1:1 support and group sessions for:

  • Young people aged 14–18.

  • Young adults aged 19–25 (no diagnosis needed, additional support needs accepted).

Assessments can often be long and difficult, so we keep our process simple. Young adults do not need a formal diagnosis to access our support.

What is Social Prescribing?

Our Mission

We support young people to make positive changes to their lives, enabling them to:

  • Reduce loneliness and isolation.
  • Build positive relationships.
  • Feel more connected to their communities.
  • Begin to feel in control of their own lives.

How Does It Work?

There is no “one size fits all” approach. As a person-centred service, we tailor our support to each young person’s individual needs and goals. We meet young people 1:1, building trust through non-judgemental support and respecting their diversity, experiences and choices.

First Meeting
Together, we agree on the most comfortable and appropriate place to meet. This may be at home for the first visit, but after that we encourage meeting in community spaces.

Identifying Support Needs
We work with each young person to explore their main concerns and the areas where they would like support.
Depending on what they need, this may include:

  • 1:1 support, creating a simple, personalised plan based on their priorities, values and motivations.

  • Small group sessions, where they can take part in our nurturing and supportive programmes.

Connecting With the Community
If it’s part of their plan, we can accompany young people to their first visits to community groups, activities or other services, making sure they feel safe, confident and welcome. We continue regular check-ins to ensure they are happy and able to engage.

How Long It Takes
There is no fixed time frame. As a guide, most young people have around 8 meet-ups with their Link Worker, though some may need fewer and others may need more.

Engagement and Readiness
Social prescribing isn’t suitable for everyone, or it may simply not be the right time. To benefit from the service, young people need to be willing to take part in self-care and self-help activities and open to making positive changes in their lives.

Who can be referred?

Young people registered with GP practices under:

  • North Hardwick and Bolsover PCN.
  • North Derbyshire PCN.

We also support young people living in:

  • Chesterfield.
  • Glossop.
  • Buxton.
  • North-East Derbyshire.
  • Bolsover.
  • Amber Valley (District and Borough Council areas).

Within these areas, we prioritise vulnerable groups, including:

  • Care Leavers.
  • Young People not in education, employment or training (or at risk of this).
  • Young Carers and Young Adult Carers .
  • Young people with Refugee or Asylum Seeker status.

How to Make a Referral

Anyone can refer a young person to the Thr1ve Social Prescribing Service.

You do not need to be a health or social care professional. We welcome:

  • Self-referrals.
  • Referrals from parents or guardians.
  • School nurses, GPs, and other professionals supporting the young person.

The most important thing is that the young person knows about the referral, understands what we do, and wants to make positive changes in their life.
