Chesterfield FC Community Trust’s Adult Wellbeing Team offers a dedicated 12-week Stop Smoking Support Programme for people living in Derbyshire. This service is available to anyone aged 18 or over who is registered with a Derbyshire GP and is ready to commit to quitting smoking.
The programme combines personalised support from a trained Stop Smoking Advisor with access to a range of Nicotine Replacement Therapy products or prescription medications. This is a full quit service that supports participants from the first assessment right through to a smoke-free future.

What the Programme Includes
- A one-to-one assessment and personalised quit plan
- Support across the full 12 weeks from a certified advisor (trained by the National Centre for Smoking Cessation and Training)
- Access to Nicotine Replacement Therapy or prescription medication, subject to GP approval
- Behavioural change support to break habits, triggers, and routines associated with smoking
- Flexible appointment scheduling to suit your availability
- Progress reviews and motivation to stay on track
Programme Structure
Week 0: Initial assessment and choice of treatment
